The importance of the dentists detects oral amyloidosis: report of three cases

Introdução

Oral lesions may be the initial manifestation of amyloidosis, and the systemic form of the disease is typically confirmed through further examination. This condition often arises secondary to various diseases, including malignant tumors specially multiple myeloma. Therefore, early diagnosis of amyloidosis is essential, particularly for making informed therapeutic decisions.

Objetivo

This study aims to report three cases of oral amyloidosis as the first sign of hematological tumors at a cancer center.

Métodos

Clinical data were collected from medical records and biopsy reports at the cancer center. Information gathered included demographic details (age, sex, comorbidities, smoking and alcohol habits, family history, and current medications), history of amyloidosis (time from first symptom to diagnosis, disease classification, and affected organs), and oral involvement (site of lesion, onset time, color, size, symptoms, and lesion type).

Resultados

All three patients (two females and one male with mean age 73,33 years), presented to the Stomatology department with a lesion in the tongue area and had no prior diagnosis of hematological malignancies. All patients presented macroglossia with lateral indentation. Oral biopsy was performed and histopathologic study displayed amyloid deposits, which were confirmed with stain Congo red and apple-green birefringence under polarized light. The patients were then referred to a hematologist and confirmed diagnosis of Multiple myeloma.

Conclusões

Oral amyloidosis is a rare condition with atypical early symptoms that can resemble other diseases. Dentists should be vigilant for clinical signs of amyloidosis to facilitate early diagnosis of oral lesions and prompt referral for appropriate treatment.
