RAZÕES PARA RECUSA DO CONTRASTE IODADO VENOSO NA TOMOGRAFIA COMPUTADORIZADA
Introdução
Contrast agents are substances used to enhance the diagnostic accuracy of imaging tests. The refusal of venous contrast use by patients is a frequently observed situation in imaging services, often for reasons that do not correspond to formal contraindications for using this material. It is relevant in clinical practice to understand the reasons why patients refuse contrast injection. Understanding patients' doubts, fears, and anxieties can help in creating informational materials in accessible language, as well as contribute to the training and education of healthcare teams.
Objetivo
To identify the reasons why patients refuse the use of venous contrast in computed tomography scans.
Métodos
This is a cross-sectional cohort study with prospective data collection, analytical, observational, and single-center, approved by the Human Research Ethics Committee (number: 6.431.089). The study population consists of patients who underwent CT scans in the Imaging Department at A.C. Camargo Cancer Center, who were indicated for the use of intravenous contrast in the exam and refused to use it.
Resultados
During the data collection period, from November 2023 to July 9, 2024, 100 patients were approached. Of these, 10 declined to participate in the study, while 90 agreed to complete the questionnaire after signing the Informed Consent Form (ICF). The most frequently cited reasons for refusing iodinated contrast in CT scans were, respectively, previous adverse reaction to iodinated contrast (n=30), renal issues (n=27), medical request (n=18), fear/anxiety (n=13). Twenty patients reported two reasons for refusal of contrast use, and four patients indicated three different reasons for refusing contrast.
Conclusões
As data collection continues, we aim to better understand the factors behind contrast refusal. This insight will be vital for creating patient-centered materials and improving healthcare team training, enhancing care and diagnostic outcomes. Preliminary results indicate that past experiences and health concerns significantly influence decisions, highlighting the need for targeted communication and education.
