THE CHALLENGES IN FAMILY DECISION-MAKING ABOUT HEMATOPOIETIC STEM CELL TRANSPLANTATION (HSCT) IN PEDIATRIC ONCOLOGY: AN INTEGRATIVE REVIEW

Introdução

Hematopoietic Stem Cell Transplantation (HSCT) is a complex therapeutic procedure, presenting numerous challenges related to family decision-making in view of its possibility of success and associated morbidity and mortality. In this scenario, there is a need for family participation in the decision-making processes, regarding care preferences and donor choice.

Objetivo

To highlight the scientific production on the challenges experienced by family members of children with cancer related to the decision-making process in the context of HSCT.

Métodos

This was an integrative review, based on the PRISMA criteria considering the PICo strategy (P = children's relatives, I= challenges in decision-making, C= Hematopoietic Stem Cell Transplantation (HSCT). Electronic databases and virtual libraries were used for the search, and the terms were validated in Mesh/PubMed/DeCS.

Resultados

The search resulted in a total of 183 publications, of which 15 - BVS, 57 - PubMed, 8 - CINAHL, 39 - Web of Science and 64 - Scopus. Of these, 43 were duplicated, leaving 140 studies. A total of 21 studies were selected for full reading. Studies point to challenges related to the mental health of family members represented by overloads/suffering, especially in end-of-life contexts. During decision-making, priority is given to meeting the child's needs and identifying the best interests of the family.

Conclusões

The centrality of the family within the decision-making process is fundamental and the best decisions are based on the sum of different perceptions, values and beliefs of those involved who add up and advocate for the benefit of the best for the child.
