CHALLENGES IN MANAGING THE SEPSIS PROTOCOL IN AN AMBULATORIAL’S ENVIRONMENT: NURSES' EXPERIENCE REPORT

Introdução

The sepsis is a condition that's results in an inflammatory unbalance aggravated by infectious processes, reflecting in high levels of mortality globally. In that sense, the implantation of clinical protocols is essential to standardize the patient's care with sepsis in the institutions. Furthermore, the technical and scientifical knowledge adequate and abilities to identify the alarming's sights and intervene with the nursing team.

Objetivo

Describe the lived experience managing the sepsis protocol in an ambulatorial care by nurses.

Métodos

This is a descriptive study experience report type, about the actuation of nurses in the management of sepsis protocol in an ambulatorial wound care in a Brazil’s cancer center, localized in São Paulo city, SP. As it is an experience repots, is dispensed the approbation of the ethical research's committee.

Resultados

Since identified vital signs or responsible level’s alteration, the nurse applies the qSOFA scale that identifies patients with suspicious sepsis or infection’s confirmation that developpe a high risk of adverse outcomes. Patients with ≥ 2 points on the scale, which covers assessment of systolic blood pressure, respiratory rate and acute change in responsiveness level, are considered positive qSOFA and need to be included in the sepsis protocol. After the medical evaluation and confirmation, the sepsis protocol starts with blood cultures collected from peripheral or central cateter. The antibiotic must be administered within one hour after starting the protocol, with vital signs checked every 15 minutes for the first hour and then every 30 minutes. In the ambulatorial flow, one of the biggest challenges is to maintain the patient's stability until transfer to emergency care, allocating part of the team to monitor the patient, in parallel with the ambulatorial care’s demand.

Conclusões

It is concluded that sepsis is a common global health problem, but it can be prevented with early diagnosis and effective assistance. In this context, the nurses play a crucial participation in the maintenance and recovery of the patient, being essential in early identification and treatment. However, in ambulatorial’s environment, the nurses face the challenge of monitoring the patient while coordinating the team to manage the demand of outpatient cases.
