BASAL CELL CARCINOMA OF THE LABIAL COMMISSURE MIMICKING SALIVARY GLAND TUMOR: CASE REPORT

Introdução

Basal cell carcinoma is the most prevalent malignant neoplasm in the population. The lesion presents a polymorphic clinical appearance, slow growth and clinical course unpredictable. Its involvement of the oral mucosa is uncommon (KIMURA et al., 2022; PERIS et al., 2023).

Objetivo

To report a basal cell carcinoma with an atypical presentation on the labial commissure.

Métodos

The work in question was prepared in accordance with the CARE guidelines (GAGNIER et al., 2014).

Resultados

A Caucasian 67-year-old male patient presented to the Stomatology department with a complaint of an asymptomatic lesion on the right buccal mucosa region with a year of duration. The patient used triamcinolone acetonide and Cicaplast® without improvement of the lesion. Medical history included Basal Cell Carcinoma in the left lower eyelid and Non-Hodgkin's Lymphoma, 10 and 8 years ago, respectively. The patient was a former smoker for 10 years.
Intraoral examination revealed an ulcered nodular lesion, measuring about 1 cm in the commissure region, extending to the right buccal mucosa, associated with a 4 mm ulcer in the skin near the labial commissure.
Diagnostic hypothesis included malignant salivary gland tumor and squamous cell carcinoma. An incisional biopsy was performed and the diagnosis was Nodular Basal Cell Carcinoma. The patient was referred to treatment and underwent resection of the lesion and reconstruction with a local advancement flap.
After surgery, the patient evolved with reduced mouth opening, extra-oral saliva leakage and swollen scar retraction. He is currently undergoing speech therapy for the management of postoperative sequelae.

Conclusões

The case in question demonstrates an atypical clinical presentation of a basal cell carcinoma on the labial commissure invading the buccal mucosa and producing a intra-oral exuberant lesion.
