Time to start treatment for patients with multiple myeloma in Brazil

Introdução

Multiple myeloma is a chronic hematological malignancy that originates from differentiated plasma cells, without definitive curative treatment, but it is treatable . The incidence of MM is 1.5 times higher in men. The average age at diagnosis ranges from 70 to 75. In the world in 2022, 187,744 new cases and 121,252 deaths were estimated. In Brazil, 4,223 new cases were treated in 2023 with a median age of 63 years.

Objetivo

To analyze the time between diagnosis and start of treatment for multiple myeloma (MM) in Brazil.

Métodos

Information was extracted from the Oncology Painel/DATASUS, from 2018 to 2023. Descriptive analyses were performed, with absolute and relative frequencies and multinomial logistic regression. The dependent variable was the time to start treatment, in three categories in days (0-30, 30-60 and 61+), the category +61 days was the reference. The variables used were Age, grouped by age group (40-59, 60-69 and 70+), Sex, Brazilian region and year of diagnosis. SPSS Statistics for Windows 27.0 software was used.

Resultados

A total of 38,051 patients were diagnosed with MM in Brazil from 2018 to 2023, 53% men, 34.9% between 60 and 69 years old and 33.1% between 40 and 59 years old. 45.7% were from the Southeast region and 56.1% received treatment in until 30 days regardless of the Brazilian region. Treatment in until 30 days was 4.2% higher in men than in women, 56.8% in the South compared to Brazil and 9% lower in patients over 70 years old compared to those under 60 years old. The start of treatment in until 30 days is 26.9% lower in the Northeast compared to Brazil and in the North region, 14.2% of patients are treated for 30 to 60 days compared to Brazil.

Conclusões

In Brazil, the shortest time to initiate treatment for patients with MM occurred in the South region. There was a greater delay in the time to start treatment in the north and northeast of Brazil. Male patients between 40 and 59 years of age were treated earlier. There is a lack of information on the staging and treatment of hematologic malignancies in the Oncology Portal and this information needs to be expanded.
