NURSING ROUND IN PEDIATRIC CHEMOTHERAPY INFUSION AT A CANCER CENTER IN SÃO PAULO

Introdução

Childhood cancer is a group of diseases that involve the uncontrolled proliferation of abnormal cells. The estimate of new cases is 7,930 between the years 2023-2025. Childhood cancer treatment is multimodal and the main pillar for treatment is the use of intravenous chemotherapy, which requires specific care and the nurse is responsible for administration, guidance and management of side effects, as well as checking the adequate functionality of venous access, aiming to interrupt and/or avoid accidents with physical or environmental risks such as overflow or chemical spills. Chemotherapy extravasation can cause irreversible damage to the affected site; while spillage leads to environmental contamination, both of which can be minimized and avoided with risk prevention measures.

Objetivo

Establish a nursing protocol during the infusion of intravenous chemotherapy for hospitalized pediatric patients with the aim of stopping and/or avoiding accidents that could cause physical or environmental damage; and reduce events related to chemotherapy extravasation and spillage.

Métodos

Qualitative descriptive study, carried out with the applicability of periodic nursing rounds in pediatric patients aged 0 to 18 years admitted to the inpatient unit, creating a nursing round protocol for all hospitalized pediatric patients who, during the infusion of intravenous chemotherapy, will receive a technical visit from the nursing professional every 30 minutes, controlled by a schedule checklist fixed on an acrylic outside the room door. During this visit, the professional must check the integrity of the bag, observing the dripping of the infusion, the route of the equipment to the venous access device, thus analyzing the permeability of the adequate infusion and minimizing the risks of adverse events.

Resultados

In 2023, we had already reached the mark of 11 spillage and extravasation events involving chemotherapy in hospitalized pediatric patients. With the creation of the nursing round protocol, the team fully adhered to the inserted protocol and since then we have noticed a significant reduction in events, reaching the year 2024 with no new reported events related to spillage or extravasation of chemotherapy in hospitalized pediatric patients.

Conclusões

The nursing round protocol during the infusion of chemotherapy in pediatric patients provided an improvement in the quality of the service, given that there were no further events related to spillage or extravasation of chemotherapy in pediatric patients, preventing accidents that could cause any harm. physical or environmental, in addition to a better experience for responsible guardians related to the safety of care.
