Navigation of onco-hematological patients and the importance of the transition of care: experience of a cancer center

Introdução

Patient Navigation (PN) has been discussed as a line of care and as one of the three important phases for reducing cancer mortality among populations in need of medical care. To this end, PN in Hematology has proven to be fundamental for the reduction of care barriers, for the improvements in regulatory processes and, with this, reduction of the journey time of cancer patients.

Objetivo

To report the experience and importance of nurse navigators of onco-hematological patients.

Métodos

An experience report on the work of nurse navigators at the Hematology Reference Center of a reference Cancer Center in São Paulo. Objective and subjective data from the Navigation Program were analyzed from May 2022 to July 2024.

Resultados

In May 2022, patient navigation by nurses at the Hematology Reference Center was implemented. Since then, more than 580 patients have been enrolled. We accounted for more than 6,000 remote consultations (telephone contacts and emails) and more than 1,300 face-to-face consultations, which demonstrates patient adherence. Over time, it was observed the need to create a bond from the first outpatient consultation with the specialty, in addition to the recruitment of patients who are admitted via the Emergency Room. Thus, the role of the Nurse Navigator (NN) in the oncological journey permeates from the diagnosis, runs through the entire treatment and directs follow-up. The NN in hematology plays an important role in the processes of care transition, as it prevents the user from getting lost in care gaps and bureaucratic processes, helping in a safe transition, in the defragmentation of care, in patient satisfaction and in the non-unnecessary hospital readmission.

Conclusões

During the two years of the nurse navigators' work in hematology, there was a gain in the reduction of barriers and in the construction of a safe, organized and constantly monitored oncological journey. In addition, navigation allowed for greater action and discussion among the care and administrative teams, in order to orchestrate care centered and directed to the onco-hematological patient.
