Título

Computerized Pharmaceutical Interventions to Promote the Rational Use of Medications in Oncology

Introdução

The clinical pharmacist plays a fundamental role at A.C. Camargo Cancer Center, achieving significant results throughout the patient journey. This professional is crucial in ensuring the safety and efficacy of prescribed medications, optimizing treatments, improving patient adherence, and promoting multiprofessional collaboration. Automated indicators are essential for effectively managing and continuously improving the quality of health services, providing accurate information and agility in analysis and decision-making support.

Objetivo

The objective of this study is to demonstrate the effectiveness of pharmaceutical interventions through a detailed analysis of medical prescriptions, presenting solid data on adherence, non-adherence, and justified non-adherence in interventions carried out during hospitalization, with an emphasis on patient safety.

Métodos

Data on pharmaceutical interventions were collected between July 2023 and July 2024 through the computerized indicators of the A.C. Camargo Cancer Center's institutional system, totaling 6,457 interventions. These interventions were categorized as adherence, non-adherence, and justified non-adherence, and occurred in the emergency department, inpatient units, and intensive care unit. The interventions were carried out based on established criteria to identify potential medication-related problems, such as admission and transfer reconciliation, venous thromboembolism prophylaxis, adjustments for renal function, monitoring of serum antibiotic levels, use of prophylactic and restricted antibiotics, dilution, stress ulcer prophylaxis, and adverse drug reactions, all aimed at ensuring patient safety.

Resultados

During the study period, there were 6,457 pharmaceutical interventions, with an average of 497 interventions per month. Most of these were accepted (5,452 - 84%), followed by justified non-adherence (769 - 12%) and non-adherence (236 - 4%), where the physician opted to maintain their conduct with justification, highlighting the exchange of information and care among the multidisciplinary team.
The main interventions carried out by the clinical pharmacy were monitoring serum medication levels (1,306) and medication reconciliation upon admission (838). These interventions have a significant impact on the treatment of hospitalized patients. Medication reconciliation upon admission ensures the continuity of chronic medication use, contributing to the reduction of failures associated with unplanned interruptions during hospitalization. Monitoring serum medication levels, especially antibiotics such as amikacin and vancomycin, helps prevent possible toxicities associated with inadequate dosages, directly contributing to the safety and efficacy of the treatments proposed to patients during their stay at the institution.

Conclusões

Pharmaceutical interventions at A.C. Camargo Cancer Center are essential to ensuring the safety and efficacy of treatments for oncology patients during hospitalization, highlighting the importance of the clinical pharmacist in optimizing patient care.
