Título

The role of the nurse navigator in the surgical journey of patients with breast cancer

Introdução

Breast cancer treatment is multidisciplinary and involves several stages and specialists. According to the immunophenotype and staging, the initial treatment may be neoadjuvant chemotherapy or surgery. The mastologist may recommend quadrantectomy, mastectomy or adenomastectomy. In cases of mastectomy, immediate reconstruction by the plastic surgery team offers physical and psychological benefits¹. The nurse navigator (NN) guides and prepares the patient to ensure the effectiveness of the procedure and minimizes future complications related to pre-surgical stage.

Objetivo

To describe the role of the NN during the pre-surgical stage for patients diagnosed with breast cancer scheduled for breast surgery with immediate reconstruction at a Cancer Center in the city of São Paulo.

Métodos

Experience report, in a navigation program at the Breast Tumor Reference Center, at a Cancer Center in São Paulo.

Resultados

Proper orientations for surgical care in breast cancer is crucial for a safe and effective procedure. The NN identifies barriers to treatment, evaluates patient understanding, and ensures compliance of preoperative steps. Ensures assessment with the anesthesiology team, preoperative breast tumor localization and lymphoscintigraphy. For surgeries with breast implants, a nursing interview and the use of a prophylaxis kit are essential, which includes the use of mupirocin on the nasal mucosa and a degerming chlorhexidine bath five days before the surgery date, aiming to reduce infections. After surgery, the NN ensures the care continuum. Between July 2023 and July 2024, 742 patients undergoing surgery were monitored, with 228 undergoing procedures with the plastic surgery team and all of them received orientations and a prophylactic kit. 112 patients who started with neoadjuvant therapy and surgery after were also included in navigation program.

Conclusões

The coordination of the NN during the surgical planning stage is essential for the patient to have a greater understanding of all the procedures involved in this type of treatment, ensuring efficiency in comprehensive patient care and consequently minimizing infections.
