Título

Salivary gland adenoid cystic carcinoma histological pattern and association with MYB-NFIB translocation and prognosis: a systematic review

Introdução

The MYB-NFIB translocation by Fluorescence in situ Hybridization (FISH) has emerged as a pivotal molecular marker in adenoid cystic carcinoma (ACC). However, the exact correlation between the presence of this genetic event, histologic pattern stratification, and its prognostic impact on survival remains unclear.

Objetivo

To conduct a systematic review of existing literature to determine the presence of the MYB-NFIB translocation detected by FISH in ACC and to investigate the association between this translocation and histological patterns and patient prognosis.

Métodos

We performed a systematic review on PubMed, Scopus, Lilacs, EMBASE and Web of Science databases, to identify retrospective and prospective cohort studies published from 2010 to May 2024. Qualitative analysis was performed.

Resultados

The presence of MYB-NFIB was detected in all studies by FISH in most of the ACC cases, with a prevalence varying of 25% to 100%. A total of 11 studies attempted to determine the prognostic importance of the translocation and association with the histological subtype and patient prognosis. Seven studies observed association with age, female sex, tumor site (minor salivary gland), histologic pattern (solid), recurrences, local relapses, metastases, perineural invasion, neurological symptoms and decrease in overall survival (OS), disease-free survival (DFS), and progression free survival (PFS).

Conclusões

The presence of MYB-NFIB translocation, detectable via FISH, strongly associates with solid ACC patterns and worse patient prognosis. These findings underscore its potential as a diagnostic and prognostic biomarker.
