Assessment Instruments in Patients Undergoing CAR-T Therapy

Introdução

The toxicities related to the treatment of hematological neoplasms present a constant challenge for healthcare professionals striving to enhance patient survival and quality of life. Physical exercise has been proposed as a safe tool used by physiotherapists during different phases of hematological treatment. However, the literature has not yet explored how physiotherapeutic assessment is conducted in patients who have received CAR-T cell therapy.

Objetivo

To identify which physical assessment tests can be performed and related to patients undergoing CAR-T therapy.

Métodos

This is an integrative review. The article search will be conducted using the following databases: Medline, EMBASE, CENTRAL, PEDro, BVS, and Scielo. Two independent reviewers will analyze the studies, with a third reviewer consulted for consensus. The inclusion criteria for the studies to be analyzed are: studies that exclusively evaluated patients over 18 years old with hematological neoplasms who underwent CAR-T cell therapy, with no restrictions on the year or language of publication.

Resultados

A total of 592 articles were identified at the end of the search, of which only 21 were selected for this review. The assessment instruments used in the articles were: EORTC QLQ-C30, FACT-G, EQ-5D-5L, PROMIS, CTCAE, SF-36, AM-PAQ “6-Clicks”, TUG test, 6MWT, 5xSTS, HCT-CT, Charlson Comorbidity Index, ECOG performance status, MDASI, MoCA, SMG, AMBACS, FACIT, Karnofsky performance status (KPS).

Conclusões

Patients undergoing CAR-T therapy may experience functional impairments that can last for weeks or even months after treatment. The use of validated assessment instruments can provide valuable information to guide treatment and prevent potential disabling functional impairments in these patients, promoting better tolerance and quality of life following CAR-T cell infusion.
