Título
Identification and management of sepsis in oncologic patients
Introdução
Sepsis is an organic inflammatory disorder in response to an infection. Oncologic patients are more susceptible to sepsis and septic shock due to the treatments they undergo. Early identification and treatment contributes to increasing the chance of survival, resulting in the need for better epidemiological and clinical understanding.
Objetivo
Estimate the proportion of cancer patients with suspected or confirmed infection who developed sepsis.
Métodos
Analytical, prospective and longitudinal observational study, involving adult cancer patients undergoing infection treatment on ambulatory care or patients admitted to hospital. Data were collected from December 2022 to March 2023, using electronic health records. For data collection, instruments standardized by the Latin American Sepsis Institute (ILAS) were used. The description of qualitative variables was done by calculating frequencies and percentages. For the quantitative variables, measures of central tendency and dispersion were calculated. The study was approved by the hospital and the Research Ethics Committee of the State University of Campinas under protocol no. 5.738.933, in line with National Health Council Resolution no. 466/ 2012.
Resultados
During the study period, the number of patient-days with cancer corresponded to 3,982. 81 infectious events were reported, distributed among 51 patients, mean age 53.84, 24 patients with hematological malignancies, and 27 patients with solid tumors, with a cumulative incidence of sepsis of 12.8%. Regarding the infectious focus, there was a prevalence of intra-abdominal infection (23.45%), followed by an undetermined focus (20.99%) and pulmonary (17.28%). Regarding blood culture samples, 30.9% showed a positive result, with a prevalence of Escherichia coli ESBL and Klebsiella pneumoniae KPC.
Conclusões
Sepsis is a life-threatening condition, especially for those with chronic diseases. In this study, the proportion of cancer patients who developed sepsis was 12.8%, with the majority being male, with solid tumors, under the clinical medical care and chemotherapy treatment.
